2019 Online Tennessee Candidate Form Requirements Instructions
All online forms allow for entry by using the tab key or your mouse. You only need to tab to the selected area and enter the information requested. A receipt acknowledgment along with a copy of the information entered for each form will automatically forward to your email once submitted.
Due Dates: The due date for each required item is referenced on the Candidate Checklist.

Section 1 - Requirements After Local Competition
Candidate and Parents Contact Info: Contact information for Parents and Candidates. Please include mailing addresses, email addresses and phone numbers along with the shipping address for items that may be sent to the Candidate prior to the State Finals. If any contact information changes, please notify firstname.lastname@example.org.
Talent Music Reservation: Required to reserve your talent music selection.
BMI/ASCAP/SESAC/GMR/PD Computer Printer (Music Clearance Documentation): These are required from BMI, ASCAP, SESAC, GMR, or Public Domain (PD) websites for the music you are requesting for your talent and submitted with the Talent Music Reservation Form. Section II - All Other Administration Requirements are Due Two at the Professional Development Training.

Section III - Additional Administration Forms
A copy of the Candidate's Birth Certificate, Driver’s License or a Government Issued Identification Card, Medical Insurance Card, Proof of Residence, Official College Transcript and/or copies of College Degrees awarded should also accompany the Candidate Contract. Please plan ahead to have these items ready to mail with the National Candidate Contract. The contract is not complete unless these items are received. Please do not fax since the information is not always legible in this format. Once approved and signed by Miss Tennessee's Executive Director, one copy of the contract will be returned to you for your files.
Resume: Please follow the instructions exactly as written.

Section IV - Photo Requirements
Candidates are to send a photo of thier chocie for the official photo. If you are using your own photographer, those photos are due by May 1st.
Headshot Photos: Need one (1) color and one (1) black & white photo submitted in digital format only. The headshots are to be emailed to email@example.com
Candid Photos: Need six (6) candid photos to be uploaded to the form site, along with a caption for each photo.
Copyright Assignment Agreements:
An Individual Photographer permission must be signed by your photographer. Agreements must be notarized. This agreement is vital. We cannot use contestant photographs in any media format until this agreement is received.
Please note: A release from your photographer MUST be emailed to firstname.lastname@example.org in order for your photograph to be used. Failure to submit this form will result in your photograph being excluded from the program, website, and all publications.

Section VI - Production Requirements
Talent Permission Letter for Original Written Material: If you have written the material for your performance, we must receive one (1) copy of a letter granting the Miss America Organization, Miss Tennessee Organization and the broadcast network permission to use your material for live on-stage performance and to broadcast.
Copyright Permission Letters: You must research and obtain the necessary clearance from the copyright owner for any talent using a literary work. A letter granting permission to use the material on-stage, along with broadcast rights must be forwarded to the State office (2) weeks before the competition. It is important to note that a dramatic performance cannot mimic a scene from a play, especially in costume.
Section VII - Program Book Information
Miss Tennessee Contestant’s PR Questionnaire: This contains specific questions that may be used for the 2019 Miss Tennessee Competition Magazine and broadcast media. Each Candidate can purchase a page to share some information about themselves. Click here to access that form.
In addition, we ask our candidate to reach out to local businesses in their community to support Miss Tennessee and their hometown candidate. You can download the Word Document form here to fill out and take to area businesses as you work to sell ad pages.
